Position: Patient Navigator
Join a dedicated team in supporting health equity and social inclusion for English-speakers in the Lower Saint Lawrence. Heritage Lower Saint Lawrence is seeking a bilingual Patient Navigator.
That person will facilitate communication between English-speaking patients and health and social service providers, provide guidance with appointment logistics, and when necessary, physical accompaniment and assistance with questions concerning lodging and transportation.
Location: Based in Rimouski (with travel throughout the Lower Saint Lawrence)
Status: Starting Part-time (14 – 21 hours)

About the Role
Reporting to the Project Coordinator, in collaboration with provincial and regional partners, the Patient Navigator accompanies English-speakers from the immediate & surrounding regions to access medical care in Rimouski and throughout the Lower Saint Lawrence.
This role provides logistical, emotional, and informational support to patients, their families and caregivers, ensuring equitable access to health services.
Key Responsibilities
Program Delivery
- Navigation and Guidance: Assist patients with appointment logistics, filling in forms, and assuring the health institution has properly transmitted medical information
- Liaison and Accompaniment: Act as a liaison between English-speaking patients and health providers; as needed, accompany patients to appointments and provide emotional support to them, their families and caregivers
- Identify and Address Barriers: Help patients overcome challenges such as transportation, lodging, language, and cultural differences
- Advocacy and Collaboration: Advocate for patients’ needs and collaborate with health professionals to ensure the continuity and pertinence of care
- Promotion, orientation and reference: create awareness of this service amongst English-speakers and H&SS service providers; refer patients to various community-based services in diverse sectors as pertinent.
What You Bring
- Bilingual (French and English), with strong communication skills
- Aptitudes in problem solving, leadership and collaboration
- Facility working with diverse institutional and community partners in Health and social services
- Excellent organisational skills, capacity to determine and manage priorities
- Empathy, cultural sensitivity, and a patient-based approach
- Proficiency with digital tools for communication, reporting, and collaboration
Qualifications
- Bachelor’s degree in Health Sciences, Social Work, Nursing, Psychology, or Community/Public Health preferred; college diploma (DEC) with relevant experience considered
- 2–3 years in health care, social services, or community support (3–5 years preferred)
- Knowledge of Quebec’s health system & regional points of service, of community organizations providing health and social services and the geographical territory, an asset
- Bilingual (French & English), strong communication and inter-personal skills, capacity to adhere to a code of ethics and preserve the confidentiality of sensitive information a must
- Valid Driver’s licence
Monitoring and Reporting
- Track patient interactions using data collection tools
- Provide regular reports to the Project Coordinator
- Participate in the evaluation and ongoing improvement of the Program
Work Environment
- The Patient Navigator will work for their local English-speaking community organization (HLSL), and collaborate with the provincial coordinator located in Québec City
- The Patient Navigator will spend the majority of their work hours assisting patients with appointment logistics, fielding phone calls and traveling to and from their region’s hospitals, clinics and health institutions.
- Location: HLSL offices-based in Rimouski and Métis, with travel throughout the Lower Saint Lawrence as needed
